Nursing homes are mainly run by either the private sector or social service agencies (SSA). The stringent advertising controls in the sector makes private companies hesitant to enter the industry. Further, these are primarily long-term residential care homes providing a range of services including daily assistance activities such as showering and cleaning, rehabilitation therapy, and nursing care and supervision. There is also eligibility required in being admitted to a nursing home, such as being immobile, physically or mentally disabled and not having the facilities to be cared for by the family or a domestic worker.


The Agency for Integrated Care (AIC) coordinates with the seniors and their caregivers to make secondary care arrangements such as home-based and community services to assist the senior, in case there is a delay in placement to a nursing home. For instance, seniors discharged from hospitals who need more time to secure their long-term care arrangements may qualify for the Interim Caregiver Service (ICS), which arranges short-term post-discharge custodial care for seniors.


Nursing care market in Singapore currently operated by both the government and private sector

The Private Hospitals and Medical Clinics Act (PHMCA) and Regulations lay speci?c guidelines for nursing homes to decide the standard and system of care, and it requires regular reporting to the Ministry. However, in the beginning of 2020, a new bill known as the Healthcare Services Act (HCSA) was set to replace the PHMCA. The new bill was introduced to streamline Singapore's healthcare regulations to adapt better to the healthcare industry's changing landscape.


Further, the nursing home sector has stringent restrictions on advertising and sales promotions. These restrictions are in place to control the encouragement of resorting to elderly care as the government believes in promoting co-residence between children and their aging parents. Moreover, there is an obligation of getting a two-yearly license from the Ministry, which will be subject to formal annual inspections.

Growing aging population spikes demand for nurses while supply remains challenged

With an expanding aging population, there is a growing demand for nurses. However, a report by the Ministry of Manpower in 2020 noted that the toughest PMET (professional, managerial, executive and technical) positions to fill were nursing jobs. One of the key reasons noted were that the salaries paid to nurses were low in comparison to other industries and the amount of mental burn-out in working long hours. Further, foreign nurses account for about one-third of the overall nursing workforce in Singapore and a recent suspension in permits of foreign workers from the Philippines has added to the shortage.

With the COVID-19 pandemic, the situation became even more challenging as nurses had to shift between quarantine, vaccination and swab operations. However, the pandemic has managed to alter the perceptions around the nursing job, as front-liners were regarded as playing a key role in the healthcare sector. This is supported through the statistics from the Ministry of Health (MOH), which indicate that the number of new nurses doubled in 2020, from 1,400 in 2019.

Singapore manages to minimize COVID-19 related deaths in nursing homes

According to the MOH, nursing home residents accounted for 0.03% of nearly 58,000 total COVID-19 cases and 14% of the 28 COVID-19 deaths in Singapore. Further, based on an approximate total of 16,000 nursing home residents, the infection and mortality rates per nursing home resident were presumed to be 0.13% and 0.03% respectively.

A recently published international study on deaths in nursing homes also featured Singapore as the second lowest among 21 countries with regard to the proportion of deaths per resident, the reasons being due to competent policy measures and stringent measures towards the isolation of resident dormitories.

Moreover, despite the pandemic, nursing homes continued to accept new admissions while supplementary precautionary measures were enacted to protect residents against the virus. This included amplified infection control and prevention practices, COVID-19 testing protocols, and safety measures for nursing home staff and residents.

Pandemic provides impetus for growth of homecare; technological scope identified to make sector more resilient

A study conducted by Oliver Wyman reported a 20% expansion in home nursing as a result of the pandemic. However, nursing homes being one of the major care segments was disrupted in almost every aspect. Visits halted and communal spaces were divided for safe distancing, and staff worked lengthy shifts to deliver coverage in the new environment.

It was recommended by the Head of Health and Life Sciences in Asia Pacific to adopt digital solutions, as she believes the pandemic has accelerated the need for digital solutions in care delivery for senior citizens. It is believed that digital technology will play a significant role, increasing choice, ensuring access to care and boosting efficiency. Processes that will fill up gaps in the existing systems can be by using remote monitoring, back-end analytics, sensor-based technology,IoT-enabled administrative systems and assistive robots. Further, social connectivity can be enhanced through digital platforms and VR-based programs.
