Disease prevention takes precedence in Singapore; healthcare screening and imaging provided by both private and public clinics

In 2017, a screening programme referred to as ‘’Screen for Life’’ was introduced, allowing patients to add on recommended screenings at private general practitioner clinics that were a part of the Community Health Assist Scheme (CHAS). Conditions such as diabetes, hypertension, hyperlipidaemia, cervical cancer, breast cancer and colorectal cancer are all listed as conditions eligible for screening. Within the first three years of the implementation of Screen for Life, 100,000 Singaporeans took part in the screening process, of whom half were individuals from low-income families.

Polyclinics offering subsidised primary care also provide screening and imaging treatments, but priority is given to follow-up patients instead of new patients. The regional health systems in Singapore have also begun to provide community-based health screening solutions in order to tackle public healthcare. Working with community partners, low-income families are targeted in order to screen for cardiovascular risks.

Private clinics, standalone screening companies, and private and public hospitals also provide other screening test options, which are not funded by the government and are covered by only selected insurance plans. These tests are generally bundled together as HSPs (health screening packages) and benefit companies.

Relaxed regulations surrounding health screening; healthcare sector introduces efficient regulatory acts to aid with sector progression

In 2019, the Screening Test Review Committee (STRC) provided medical practitioners with clinical guidance, despite the health screening sector in Singapore not being strictly regulated. As per the Singapore Medical Council's (SMC) Ethical Code and Ethical Guidelines published in 2016, screening tests should be validated and only used in clinically appropriate treatments.

A legislative act referred to as The Private Hospitals and Medical Clinics Act, which was introduced in 1980, is said to be replaced by the Healthcare Services Act in order to improve regulations and governance within the sector. This new Act was said to be implemented in January 2022 and provide regulations for hospitals, primary care clinics and private screening services.

Health screening and annual checkups suspended temporarily during pandemic; doctors urge patients to prioritise routine health checks

Due to the circuit breaker restrictions that were implemented in Singapore during the COVID-19 pandemic, annual medical checkups, which included health screening and imaging, were suspended on a temporary basis. The Government instead implemented an essential services list, which excluded health screening and imaging.

But as the government gained control over the pandemic, patients began to opt out of rescheduling health screening checks due to the fear that the coronavirus pandemic had left upon the community. Patients feared the idea of detecting life threatening illnesses although many doctors have advised early screening, which could lead to interventions.

Doctors proceeded to encourage patients to reschedule annual health checks and opt for general screening tests that would include the checking of cholesterol, sugar, blood pressure as well as common cancers. In 2018, 52% of deaths in the country were due to cancer, 28% due to coronary heart illness and 6% due to strokes.

The circuit breaker resulted in many families being stuck at home, with little to no chance of exercise and an increasing amount of stress brought on by the pandemic. In order to combat the negative impacts of COVID-19 on people’s health and also motivate individuals to opt for preventative care, doctors have keenly encouraged the citizens of Singapore to take more interest in fitness and to opt for preventative measures.

Singapore becomes first in the APAC region to access HSPs for brain health; cognitive health screening assessments being enabled for cognitive decline

In June 2022, a leading hospital in Singapore, Parkway Shenton, became the first healthcare provider to offer digital brain screening and monitoring in the APAC (Asia Pacific) region. The initiative assists patients with severe cognitive decline, strokes, Long COVID brain damage, dementia and Parkinson’s disease.

As part of the process, digital imaging would utilise Neurowyzr’s proprietary technology, referred to as Digital Brain Function Scan. The company leverages neurosciences that are based on digital solutions and is a quicker and more affordable option for patients. The scan is to be made available via an online portal, only upon prescription by physicians. The Digital Brain Function Scan reduces the need for multiple visits to the clinic as it can be connected to a device such as a mobile phone, iPad or laptop and can assess the patient within a matter of minutes.

As per the instructions of the World Health Organization (WHO) and leading neuroscientists, the need for brain cognitive screening is highly recommended as studies show that nearly 10% of those in their 30’s experience brain health problems. Further studies have also shown that those suffering from non-communicable diseases such as metabolic syndrome, obesity, diabetes, hypertension, high lipids and high cholesterol are at higher risk of brain damage.

Bioethics cause increasing concern; HSPs prove detrimental to individuals seeking treatment

In contrast to general clinical services, health screening and imaging is carried out on individuals with zero symptoms. The green light to proceed with screening is usually provided by doctors and health authorities in the case of suspicions. Yet, the topic of ethics is brought up due to the screening process being one which may lead to an asymptomatic individual developing symptoms and health conditions post-screening. In Singapore, HSPs have become more prevalent and can be accessed via GP clinics offering the Screen for Life national program.

The main purpose for screening is due to population benefits instead of individual benefits and this has resulted in a negative impact on the sector. Conflict revolving around ethical frameworks and the fact that screening tests are not 100% sensitive and specific have resulted in dire concerns.

Within the city, HSPs are provided for individuals within the working age group - between 20 and 64 years. The inclusion of HSP packages with non-recommended tests is unfavourable and shows a greater risk to the patient than benefit. HSPs could also bring about issues revolving around the distribution of resources as demand increases and could lead to patients being exposed to unnecessary harm.

Public education on screening has been suggested as a means to increase awareness about HSPs and the tightening of regulations in order to minimise the use of non-recommended tests being implemented. Many individuals raising awareness also advise limiting the provision of screening services in all hospitals and instead, advise initiating the appropriate allocation of healthcare resources for the safe handling of tests.
