The Healthcare sector in Singapore comprises four main segments: Hospitals, Western Clinics, Non-Western Clinics, and Dental and Other Medical Services. Government hospitals account for 80% of all hospital beds in Singapore, while the private sector accounts for 20%.
Singapore General H ospital was ranked the third best in the world by Newsweek magazine in 2019. Joint Commission International (JCI) has accredited 13 Singapore hospitals and healthcare facilities, and international medical research bodies such as the American Association for Cancer Research, Duke University, and the Healthcare Information and Management Systems Society. JCI has also established a presence in Singapore to advance drug discovery and develop therapies for unmet healthcare needs. As a result, Singapore attracts over 350,000 medical tourists each year, on average.
Singapore hosts over 60 multinational medical technology (MedTech) companies, with a strong presence of more than 25 research and development (R&D) centers established by these companies, in addition to a local pool of over 220 MedTech start-ups and small and medium-sized enterprises (SMEs). Over 50 global MedTech firms have their regional headquarters in Singapore, which indicates the strategic importance of the country, serving as a hub for expansion to Asian markets.
Strong government spending and well established funding system support sector growth
The Singaporean government has allocated a USD 5.6 billion budget for short- and long-term infrastructure development, and to support subsidies for the poor. Infrastructure development includes healthcare infrastructure, healthcare delivery, and managing the associated costs and issues related to an ageing population. The budget also includes larger subsidies for surgical implants, the treatment and management of chronic diseases, and funding programs to promote healthy and active ageing programs.
Currently, more than 12 telemedicine start-ups operate in accordance with Singapore’s Ministry of Health clinical and data governance, with rapid adoption by healthcare organizations. Key players include Doctor Anywhere, MyDoc, HiDoc and Speedoc. Singapore’s three main mobile operators - Singtel, M1 and StarHub - have also collaborated with the government to provide connectivity to elderly monitoring functions as part of smart-home projects in public housing.
The country provides universal coverage for its citizens through a multi-payer funding system that includes tax revenues and payments from individuals and their employers, through mandated life insurance deductions, and the Central Provident Fund. The system is centered on three programs commonly known as the ‘3Ms’ – MediSave, MediShield Life and Medifund.

Healthcare sector spend to double by 2029, driven by government spending and ageing population
According to Fitch Ratings, Singapore’s healthcare market is expected to grow from SGD 24.5 billion in 2020, to SGD 45.9 billion by 2030, and healthcare spend as a percentage of GDP is expected to reach around 9% by 2029, from 4.46% in 2018.
The envisaged growth is driven by the following factors:
- Rising government spending on healthcare
- Growing local consumption of healthcare services among the ageing population and
- The trend towards earlier diagnosis of chronic conditions, close monitoring and follow-up.
Government healthcare expenditure is estimated to grow to USD 36 billion by 2029, while private healthcare expenditure is forecast to reach USD 13.5 billion.
Singapore‘s ageing population, currently accounting for 10.7% of the total (the highest among all ASEAN countries), is estimated to increase rapidly over the next 20 years, reaching over 25% of the population by 2030. The country’s hyper-ageing population is expected to create a significant demand increase for geriatric medicine, home-based patient monitoring, memory care and rehabilitation medicine. Therefore, the government has increased investment in medical infrastructure development such as nursing homes and rehabilitation centers.
The trend indicates that patients will have to manage their own health amid the shift towards telemedicine and electronic records management. Moreover, health management, health screening, disease management, preventive care products, access to homecare resources, and smart/assisted home facilities are drawing investments for R&D.
This also creates the risk of cybercrime given the increasing amount of sensitive personal data being carried over operators' data networks for diagnoses. ‘Health tech’ solutions that focus on cybersecurity, electronic health records, data sharing and distributed ledger technologies are key growth opportunities.
Expansion of digital health and medical devices to boost sector growth
Singapore is seeing an increase in the deployment of digital health, with the aim of enabling early-stage interventions that can improve health, driven by technology. The country' thriving start-up ecosystem of digital health providers has been created by its robust regulatory guidelines, considered among the best in Asia.
Around 60% of the world’s microarrays, and one-third of thermal cyclers and mass spectrometers, are manufactured in Singapore. The city-state’s medical devices market is expected to grow further, with the launch of the SG Patent Fast Track Programme in 2020 by the Intellectual Property Office of Singapore (IPOS), which aims to approve patent applications in as quickly as six months, thereby cutting the time to file a trademark by 80 days.
Imports of medical equipment in 2020 saw a 6% growth compared to 2019, with imports from the US accounting for 22%. On average, more than 70% of products imported into Singapore are subsequently re-exported. Adoption of the ASEAN Medical Device Directive (AMDD) by the member countries is expected to open Singapore’s device export market to a population of 600 million in the region, with the uniform classification criteria for medical devices.

- Remuneration, and the Purchase of Goods and Materials, are the two largest cost elements in the sector, making up more than 60% of operating expenditure in 2019.
- In terms of profitability, Western Clinics generated the highest profitability ratio of 25.1% in 2019, followed by Non-Western Clinics (22.2%), Hospitals (8.2%), and Dental and Other Medical Services (4.7%).
